Initiating a Claim
Below are the steps to initiate a claim for employees.
1. Notify your Manager and Leave Administrator
If possible, notify your supervisor at least 30 days prior to your leave request, via email. Include the start and end date of your anticipated leave request, as well as the best method for contacting you during your leave.
Notify the Leave Administrator of your leave by submitting your leave request electronically at the USC Leave Request button below. Once notified, the Leave Administrator will contact you regarding the next steps required to process your request.
2. Contact Broadspire
Contact Broadspire, USC’s third-party administrator, to initiate your claim. Broadspire can be reached at (800) 495-2315. They will send you additional information that should be provided to your physician for completion and then returned to your designated claim examiner.
3. Claim Verification
In order to successfully process your claim, you must provide certification, confirming your start and end date of leave to our office.
Please note: you are required to update Provost HR and Broadspire with documentation and any changes that may occur, separately.
4. Contact the State of California
Note: If you participate in the USC disability program, DO NOT initiate a separate claim with SDI
If you participate in the State of California Disability Plan, contact the State of California SDI program to initiate your claim for benefits. You may either apply online with SDI or call (800) 480-3287.
