Employees are fully responsible for submitting accurate and timely certifications for their leave request.

If you have not been released back to work:

Submit a continuation certification of leave immediately to both Broadspire and the Leave Administrator .

If you have been released back to work, with restrictions:

Submit a certification of return to work, listing the restrictions to the Leave Administrator, at least one week prior to your scheduled return to work date.

If you have been released back to work, to full duty:

Submit certification of return to work to the Leave Administrator, at least one week prior to your scheduled return to work date.
